How do i write this into piano tabs?

I need to convert this to standard piano format:

When converted is should be like this: 3|--a--d--

2|--a--a--. I have no idea how to do this...looked online how to do it....nothing helps....please someone write out the piano tabs out for this. YOU HAVE NO IDEA HOW MUCH THIS MEANS TO ME :)
The second line is the B string not a D



so copy and paste this:
